Type 1 Opt Out: The way NHS Digital handles some patient data is changing on the 1st of July. You can find more information here. If you want to "opt out" of NHS Digital using your data in this way you must complete this form and return it to us before 23rd June 2021. 


Dr Claire Laband Sadly our GP Dr Claire Laband has been diagnosed with pancreatic cancer. She has been a much loved GP partner at Cheddar Medical Centre for many years, and was already planning to retire later in 2021. She has now had to leave her clinical work abruptly in order to undergo treatment. For those patients who would like to pass on their good wishes to Dr Laband, please send emails to Cheddarmc.chedmed@nhs.net with the subject "Claire Laband Retirement". We will ensure these good wishes are passed on to her.

Community Midwives

Midwife- Emma Ratcliffe

Visits expectant mothers in their homes, holds a weekly antenatal clinic, holds relaxation classes and cares for mother and baby after delivery. If you need a midwife urgently out of surgery hours, please ring 01278 436774 (Mary Stanley Midwifery Led Unit, Bridgwater Hospital Maternity Unit) and they will bleep the midwife on call.  If you have recently discovered you are pregnant and have not yet been referred to the midwife, please complete this Midwife Referral Form, print it out and return it to us at Cheddar Medical Centre.  (Forms are available from reception if you do not have access to a printer).

Community Nurses

Our Community Nursing Team give nursing care to those patients unable to visit the surgery. They are based at Cheddar Medical Centre and can be contacted on 01934 743519 where, if they are unavailable, a message can be left on their Voicemail.

Health Visitors

Health Visitors are primarily involved in the provision of advice, health education and developmental screening for children under 5 years. They are based at Axbridge Surgery and can be contacted on 01934 732425.
